Medical Forms and Notarization
To ensure a safe and successful camp experience for every participant, all campers are required to submit completed medical forms prior to attending RYLA. These forms help our staff and nurse respond appropriately in the event of illness, injury, or emergency.
We know paperwork can feel overwhelming (especially when it includes notarization), so we’ve outlined everything you need to know and broken down into steps.
Completed medical forms are due June 1, 2026.
Step 1: Fill Out the Forms
Download and print the forms. Fill them out completely and carefully (pen is preferred).
⚠️ Important: Do not sign the form ahead of time. Forms must be signed in front of the notary.
Step 2: Get Forms Notarized
A notary public (often just called a notary) is a person who is authorized by the state to serve as an official witness when certain documents are signed. Their job is to verify identity, confirm you understand what you are signing, and ensure the document is signed willingly. A notary does not review of judge the content of the form or decide whether information is correct. Their role is simply to witness the signature and apply an official stamp or seal.
Notaries are commonly available at:
- Banks or credit unions
- Courthouses or government offices
- Some libraries
- Shipping or office service centers (such as UPS or similar stores)
Most notary services are free or low-cost. Calling ahead is always a good idea.

Why does RYLA require notarization?
Some medical and liability forms require an added level of verification to ensure they are signed by the correct person and completed intentionally. Notarization helps protect both campers and RYLA by confirming that the required signatures are valid.
This is a standard requirement for many youth programs and camps.
What to Expect at a Notary Appointment
A notary appointment is usually quick, just a few minutes.
You will need:
- Your unsigned medical forms
- A valid photo ID for the person signing, who in this case is the camper’s parent or guardian. Valid forms of ID usually include driver’s license or state ID.
The notary will:
- Check the signer’s ID
- Watch the form be signed
- Apply their stamp or seal
And that’s it! You’re done!
Step 3: Scanning Your Documents (from home!)
Once your camper’s medical forms are completed and notarized, you’ll need to scan them so they can be submitted electronically.
You do not need a scanner to do this. A smartphone works great.
We recommend using a free scanning app, such as: Adobe Scan, Microsoft Lens, DocScan, Apple Notes, or Google Drive’s Scan Feature on mobile. You’ve got options! These apps allow you to take clear photos of each page and automatically convert them into a single, high-contrast, easy-to-read PDF.
Please make sure all pages are fully visible and easy to read before saving.
Step 4: Submitting Your Forms Securely
All completed medical forms must be uploaded to RYLA’s secure Jotform submission link.

- only authorized RYLA leadership staff can access these documents
- Forms are encrypted and stored securely
